The Trump administration's newly appointed health care affordability czar made a direct appeal to hospital executives this week, urging them to embrace significant cuts to Medicaid as a necessary step toward fiscal responsibility. Speaking at a closed-door meeting with top hospital leaders, the official argued that reducing federal spending on the program would force providers to become more efficient and lower overall costs for patients.
"We cannot keep pouring money into a system that rewards volume over value," the czar said, according to two attendees who spoke on condition of anonymity. The official emphasized that the cuts, which could total billions over the next decade, would ultimately benefit taxpayers and reduce the national debt.
Hospital executives in the room expressed deep concern, noting that Medicaid already reimburses them at rates well below the cost of care. Many said they are already struggling to keep emergency rooms open in rural areas and are bracing for a wave of uncompensated care if the cuts go through. Several leaders told the czar they are working to mitigate the impacts by scaling back administrative costs and expanding partnerships with community clinics, but warned that major reductions could lead to service closures and longer wait times for low-income patients.
The proposal has drawn sharp criticism from patient advocacy groups, who argue it will shift the financial burden onto states and hospitals already stretched thin. The czar countered that the administration is open to phasing in the changes and offering waivers for states that demonstrate innovative approaches to care delivery. No timeline for the cuts has been finalized, but the meeting signals that the White House is moving forward with its push to reshape the health care safety net.
